The Oregon State Hospital in Salem, pictured this winter. Leaders of the hospital told lawmakers they are focused on improving its operational and cultural issues, but couldn't fully explain troubling actions by former hospital leaders.

Leaders of the troubled Oregon State Hospital told state lawmakers on Wednesday that they can’t fully explain why major, prolonged issues at the institution under its former leaders went overlooked or ignored for years.
