We’re reaching out to you with an exciting opportunity to directly influence the future of Southern Coos Hospital & Health Center!

As we develop our Master Facilities Plan, a crucial part of this process is gathering input from our community and dedicated staff. Your insights and experiences are invaluable as we envision the best possible environment for our patients and our team. We truly value your opinion and encourage you to participate!

There are two convenient ways to share your thoughts:

  • Easy Digital Survey: Simply scan the QR code on the attached flyer with your smartphone to access our anonymous survey (unless you choose to provide your contact information at the end). This is a quick and easy way to contribute!
  • Print and Drop-Off: Alternatively, you can download and print the attached survey, fill it out at your convenience, and return it to a designated collection box located at the main hospital front admitting desk or at the clinic front desk.
