We’re Hiring: Patient Care Coordinator 🩺💙
The Patient Care Coordinator plays a key role in delivering excellent customer service while supporting patients, families, providers, and our care team. This position assists with patient intake, scheduling, referrals, insurance verification, and coordination of care—helping ensure every patient receives timely, compassionate service.
What you’ll do:
Serve as a welcoming point of contact for patients and referral partners
Process new referrals and maintain electronic medical records
Coordinate start-of-care and daily schedules for clinical staff
Verify insurance benefits and obtain authorizations
Act as a liaison between patients, families, and clinicians
What we’re looking for:
Strong customer service, organization, and communication skills
Healthcare or home health/hospice experience preferred
Comfortable with medical terminology and computer systems
📍 Location: Crescent City, CA
🕘 Schedule: Monday–Friday, 8:30am–5:00pm
